Commissioning should confirm readiness and performance; it should not be the first stage capable of revealing that materials are incorrect, interfaces were not coordinated, inspection records are missing, or installations were executed outside requirements.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When a large share of failures appears only during final testing, there is usually an earlier gap in quality governance. The problem may lie in ambiguous requirements, insufficient PIT\/ITP, poorly positioned inspection points, weak traceability, physical closeout before verification, NCRs treated only administratively, or the absence of objective readiness criteria for releasing a system to testing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Prevention requires a continuous chain of evidence. Each stage should have its own entry criteria, inspections, records, nonconformities, corrections, and exit conditions. In this way, commissioning receives technically prepared systems \u2014 rather than an apparently completed project that still needs to discover its actual condition during testing.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why nonconformities reach commissioning late<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A nonconformity may exist for weeks or months before it is noticed. This happens because physical execution and technical verification do not always progress at the same pace. A work front may be closed, energized, or internally handed over before all records have been reviewed or before the critical requirement has actually been tested.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The main mechanisms that push problems to the end are well known: missing acceptance criteria at the source, generic inspections, incomplete evidence, release under schedule pressure, interface failures, and a culture of \u201cfixing it during commissioning.\u201d This last behavior is especially dangerous because it turns commissioning into late quality control.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td>Upstream cause<\/td><td>Effect observed during commissioning<\/td><td>Typical consequence<\/td><\/tr><tr><td>incomplete requirement<\/td><td>test without clear criterion<\/td><td>dispute over pass\/fail<\/td><\/tr><tr><td>outdated drawing<\/td><td>incompatible installation<\/td><td>rework and new as-built<\/td><\/tr><tr><td>material without traceability<\/td><td>uncertainty about specification<\/td><td>documentary hold or replacement<\/td><\/tr><tr><td>superficial installation inspection<\/td><td>functional failure during testing<\/td><td>dismantling and correction<\/td><\/tr><tr><td>NCR without technical closeout<\/td><td>deviation reappears<\/td><td>retest and delay<\/td><\/tr><tr><td>uncoordinated interface<\/td><td>subsystems work only in isolation<\/td><td>failure in integrated testing<\/td><\/tr><tr><td>instrument without valid calibration<\/td><td>questionable result<\/td><td>repeated measurements<\/td><\/tr><tr><td>poorly classified punch list<\/td><td>blocking item reaches testing<\/td><td>sequence interruption<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">The first preventive measure is to recognize that \u201cbuilt\u201d and \u201cready for commissioning\u201d are different states. Readiness requires physical condition, documentation, safety, configuration, instruments, and open items compatible with the test to be performed.<\/p>\n\n\n\n<figure class=\"a3a-mermaid\"><svg
